The patient-centered medical home model of care is designed to provide patients with a comprehensive, coordinated approach to primary care which in turn leads to improved quality and lower medical costs.

The core goals of the Patient Centered Medical Home are to enhance the quality of care you receive, to lower costs,and to keep you healthy

Physicians’ Organization of Western Michigan, West Michigan Physicians Network, and Zeeland Community PHO provide support to physician practices, including those striving to become a Patient Centered Medical Home.
